ATP LISTING DIRECTORY AMENDMENT FORM

From 01/02/2024 any alterations to your directory entries will be charged at £5.00 per listing through your ATP GoCardless arrangement.

Please read this guidance in accordance with the

🔗 'ATP Directory and Listings Policy': ATP Constitution Appendix 3.

The ATP will not share any personal identifiable data or documents outside of the organisation without your agreed consent. These directory listing requirements are to ensure that the directory remains current and up-to-date.

This form is to make alterations to your current listing(s) only, ONE form per directory listing entry please.

Please note it is each individual MEMBER'S RESPONSIBILITY to regularly check their listing for accuracy, along with searches (location and name) working as desired.

The ATP directory is a list of services to help parents find practitioners local to them, rather than an advertising service for members. It is provided at a low cost to members of the ATP, and is run by volunteers.

I confirm that the listing of this service into the ATP directory is a legitimate representation of the service and location offered, and has been completed by myself as a registered member of the ATP and not a representative of myself (unless with committee approval). I understand that providing false information is both deception and fraud and would be in breach of professional standards and my code of conduct.

Use of the ATP logo is prohibited without written ATP Committee consent (see Constitution part 8). Use of the members logo is freely available for download and display on your websites/merchandise from the members area.
